Patient Registration Forms

If you would like to become a patient of Texas Family Medicine, please complete the New Patient Registration Forms online found below. After completing these forms, call our office at (214) 387-8288 to schedule an appointment or click here to request an appointment.

Please arrive at your first appointment 15 minutes early and bring your Drivers License and Insurance card.

Once you become a patient, you can enroll in our Patient Portal, an easy to use and secure Web site to manage your healthcare needs. It includes online features such as e-mail communication with our staff, obtaining test results and the ability to request appointments.
